A printable ID card for a child or adult whose medical or mental health condition can cause confusion, trouble communicating, or behavior that may be misread.
Your photo and everything you type stay on this device. Nothing is saved, sent, or tracked. When you close this tab, it is gone. You choose what to share, and you can leave the diagnosis off the card.

The code saves a contact on this device: the emergency contact's name, the phone numbers, who the card is for, the second contact, and the notes below. It does not include the photo, diagnosis, medications, or doctor. Nothing is sent to a website.
The small code on the card is the one that prints. Test it with the button under the preview.
In the print box, choose 100% or Actual size. Do not use Fit to page.
No. The card is made entirely in your browser. Nothing you type or upload is sent anywhere, stored, or tracked. There is no account and no analytics. When you close the tab, it is gone.
Someone helping you, like a police officer, paramedic, teacher, or stranger, can point their phone camera at the code. Their phone offers to save your emergency contact's info plus the extra notes you wrote, so they can call your person and understand how to help. No app or internet is needed, nothing is tracked, and nothing goes to a website. If your info changes, make a new card.
Yes. Uncheck “Include a QR code on my card.” No code is printed, and the back of the card uses the full width.
No. You choose what to disclose. You can describe what helps without naming a condition, and you can leave the diagnosis off the card.
No. It is a helpful card, not a medical ID or a legal document. Consider a medical ID bracelet too, and keep a copy in your phone's emergency Medical ID.
